Care Coordination

A team of nurses, social workers, and care coordinators will be employed to help check on your care. They may call you after an appointment or a procedure to make sure you understand how to take your medicine or schedule follow-up visits. They will also share information with your doctor to make sure you get the right care. Our Care Coordination team will also provide you with education regarding your specific illness or treatment plan and may be able to assist those who need non-medical assistance.
